Please note:
Maple Eye Care requires payment at time of service. If you have medical/vision insurance we are happy to work with your carrier to maximize your benefits and will file claims directly with them for reimbursement of services you receive, but all co-pays and out-of-pocket expenses are ultimately your responsibility.

*Medical procedures and imaging may be subject to deductibles. You may be charged at the time of the procedure, or billed after your insurance company has settled with us. Either way, you are ultimately responsible for the cost of your treatment.
Pupillary distance (PD) measurements are not part of the eye exam. PD’s are measured by an optician when you purchase glasses. We charge $10 for a PD measurement. This fee is waived with a purchase of glasses from our office.
Return Policy:
If you have any issues with the glasses that you purchased with us, please call us within 3 weeks of purchase. We are happy to work with you to make it right, whether it is a simple adjustment of the frame or a modification of the prescription. We can re-make your lenses once at no cost within 30 days of purchase. You can return/exchange your frame within 30 days of purchase for a refund minus a 10% restocking fee.
If you purchased contact lenses from us, you can return any unopened/unmarked/not expired boxes for an exchange towards a new prescription.
